By eliminating the need to pay all the overhead created by processing insurance claims, we can offer you and your family an excellent value for the healthcare you receive with us at The Tree of Life Clinic. In addition to our fair rates, we have special pricing for routine labs, imaging studies, vaccines and prescription medications right in-house.

Membership Pricing

  • The Tree of Life Clinic 0-25 years $25/month

    $25/month (children with an adult membership)

  • The Tree of Life Clinic 19-44 years $89/month

    19-44 years $89/month

  • The Tree of Life Clinic 45 - 99 years $109/month

    45 - 99 years $109/month

*Members under 18 accepted with a parent/guardian adult membership

**Currently, we are not able to take Medicare or Medicaid patients. Medicare patients should inquire if interested because we hope to take Medicare in the near future.

***Billing can be done annually or monthly. HSA/FSA deductions may qualify.

Appointments that are not cancelled more than 24 hours before the visit will incur a $25 fee.

Enrollment Fee:

Membership enrollment is for a one year minimum.

$99 per household due at sign-up.

$200 re-enrollment fee
